Traveller Pre-Departure Form
- All travellers arriving into Malaysia are required to complete the Traveller Pre-Departure Form (TPDF) through ‘Traveller’ icon on the MySejahtera application (Click the Travel For xxx (name of the traveller)) and upload the results of RT-PCR or RTK-Ag Professional (for travellers with recent post-COVID infection or travellers from Singapore) test done within 2 days prior to departure.
- Upon submission of Pre-Departure Form and upload of the pre-departure test, a BLUE Traveller’s card (if fully vaccinated) or RED Traveller’s card (if unvaccinated/ partially vaccinated) will appear on the app. Fully vaccinated travellers who travel from Singapore via Land will receive PURPLE Frequent Traveller’s Card.
- Partially or unvaccinated travellers will be issued with a Home Surveillance Order (HSO) for 5 days quarantine upon arrival.
